I understand that by signing this agreement, I am assuming full legal ownership and lifelong responsibility for this dog upon written approval by Rescue Dog Network.

Ownership of the dog transfers to the Adopter upon: Approval by Rescue Dog Network, AND Payment of the agreed adoption fee, AND Written confirmation from Rescue Dog Network. Until that time, the dog remains property of Rescue Dog Network.

Adoption fees are non-refundable unless otherwise stated in writing by Rescue Dog Network.

Adopter acknowledges that: The dog’s known medical history has been disclosed. No guarantee is made regarding temperament, breed, longevity, or future health. Veterinary care is the sole responsibility of the Adopter upon transfer of ownership.

Adopter agrees to: Maintain routine veterinary care. Keep vaccinations current as required by law. Maintain heartworm prevention (if applicable). Seek prompt medical attention when needed.

Adopter agrees the dog will: Be kept as an indoor household companion. Not be chained, tethered, or permanently kenneled outdoors. Not be used for breeding. Not be used for illegal activity. Not be trained for fighting, guarding for criminal activity.

Adopter agrees to provide adequate food, water, shelter, training, and humane treatment at all times.

The dog may NOT be: sold, rehomed, given away, surrendered to a shelter If the Adopter is unable to keep the dog for any reason, the dog must be returned to Rescue Dog Network. Rescue retains first right of refusal and reclamation.

Rescue Dog Network reserves the right to reclaim the dog if there is evidence of: neglect, abuse, breach of contract, false statements made in this agreement. Adopter agrees to cooperate with any welfare check if credible concerns arise.

Adopter assumes all risks and liabilities associated with ownership and agrees to indemnify and hold harmless Rescue Dog Network, its volunteers, officers, and affiliates from any claims, or damages, or legal actions arising after transfer of ownership.

By submitting this document, I acknowledge: Adoption is a lifelong commitment. I have had sufficient time fostering this dog to evaluate compatibility. I am choosing to permanently adopt this dog.

This document constitutes a legally binding agreement under the laws of the State of Florida. Electronic signatures are considered valid and enforceable.
